DATA SUBJECT APPLICATION FORM

Under the Personal Data Protection Law No. 6698 (“KVKK”), personal data owners defined as data subjects or their legal representatives (“Applicant” or “Data Subject”) have the right to make requests regarding the processing of personal data as stipulated in Article 11 of the KVKK.

Pursuant to the first paragraph of Article 13 of the KVKK, applications regarding these rights must be submitted to us in writing or by other methods determined by the Personal Data Protection Board (“Board”) by Odeon Turizm İşletmeciliği Anonim Şirketi (“Odeon” or “Company”), which is the data controller. Depending on the nature of your request, it will be processed free of charge as soon as possible and within thirty days at the latest. However, if the process incurs additional costs, a fee may be charged in accordance with Article 7 of the Communiqué on the Procedures and Principles for Applying to the Data Controller.

APPLICATION METHOD

● Application by Mail or in Person:  The Application Form or petition below, completed and signed by the Applicant, along with documents proving their identity, may be submitted in person or by mail to the address Ayazağa Mahallesi Azerbaycan Caddesi 2B No: 3K/27 Sarıyer/İstanbul.

● Application by Registered Electronic Mail (KEP): The application can be made by sending a signed petition or, if preferred, the “Application Form” from the applicant's registered electronic mail (KEP) address to [email protected]. 

● Application via Email Using Mobile Signature or Secure Electronic Signature: The Applicant may apply by sending an e-mail to [email protected] with a petition signed with a mobile signature or secure electronic signature or, if they wish, by filling out the “Application Form”.

● Application via Email Using the Email Address Registered in the Data Controller's System: The relevant person can apply by sending an email to [email protected] using the email address previously reported to the data controller and registered in the data controller's system. In this application, Odeon reserves the right to request additional identity verification depending on the subject of the request.

Please note that you will receive a faster response if you write “Request for Information/Change under the Personal Data Protection Law” on the envelope or in the subject line of your email.

Additionally, applications may be submitted via a Notary Public or other legally valid methods that enable identity verification. It is recommended that applicants to comply with the above-mentioned requirements to the extent possible, as this will increase the likelihood of a positive and timely outcome.

The application form below has been designed to ensure that your requests are addressed accurately, completely, and within the timeframe specified by law. Odeon reserves the right to request additional documents and information (such as a copy of your ID card or driver's license) for identity and authorization verification in order to eliminate legal risks that may arise from unlawful or unjust data sharing and, in particular, to ensure the security of your personal data. Odeon shall not be liable for any requests you submit via the form if the information provided is inaccurate or out of date, contains false/misleading information, or is submitted without authorization. In such cases, Odeon reserves the right to reject the request, stating the reasons for doing so.
